Our team is passionate about helping participants, who live with disability improve their quality of life and live as independently as possible.

Your Life Disability Support understands that every person is unique and requires tailored services to suit their needs, we will help assist you to live as independently as possible. Our well-trained staff can assist you in various daily personal activities. These supports can be provided across a range of environments including your own residence, living with family or other people, when undertaking social, recreational, education or employment activities, and during holidays away from home. NDIS outlines that these kinds of activities require support or supervision as followed:

  • Personal hygiene, including showering, bathing, oral hygiene, dressing and grooming;
  • Toileting, bladder and bowel management and general care;
  • Eating and drinking;
  • Attending appointments;
  • Use of aids and appliances, hearing and communication devices;
  • Mobility and transferring, for example moving in and out of bed and on or off the toilet

Your Life Disability Support complies with the NDIS Practice Standards for High Intensity Daily Personal Activities. Our support workers and registered nurses are extensively qualified. These supports can be provided across a range of environments like your own residence, living with family or other people, when undertaking social, recreational, education or employment activities, or during holidays away from home. NDIS outlines these kinds of activities require support or supervision as followed:

  • High intensity support activity for Complex Bowel Care. It requires a specialist level of support where a person is at risk of severe constipation or fecal incontinence.
  • High intensity support activity for Enteral Feeding and Management. Our qualified workers can provide support to a person who is reliant on Percutaneous Endoscopic Gastrostomy (PEG). We also provide support to a person reliant on NG feeding.
  • High intensity support activity for Tracheostomy Care. Our workers are qualified to provide support to a person who is reliant on ventilation.
  • High intensity support activity for Urinary Catheters. This involves replacing and disposing of bags and monitoring health of people using indwelling and suprapubic
  • High intensity support activity for Ventilation. We are qualified to perform ventilation related various tasks like – hygiene maintenance, assemble components, ventilator operation procedures etc.
  • High intensity support activity for Subcutaneous Injection. This kind of task requires a worker to have special competency to provide support to a person who is unable to self-administer injections.
  • High intensity support activity for Diabetes Management. Our support staff can assist you to take regular injection as part of diabetes management plan if you are unable to do so.

Other high intensity support activities are – management of high risk of seizure, pressure care & wound managements, mealtime preparation & delivery, stoma care etc.

We understand that participants are in constant care of support workers, therapists and medical practioners.

With this understanding Your Life Disability Support, have designed, catered and tailored programs to suit participants needs in a social environment. We encourage participants to partake in social and recreational group activities to enhance their quality of life and stimulate social interaction.

We will help you explore various recreational activities to help identify your personal interests. This may involve sports, exercise, games, cooking etc. We are passionate about helping you integrate into the community just like any other person. This will involve enjoying and attending community festivals/services and events which will help improve your interpersonal and social skills.

Your Life Disability Support knows that managing NDIS plans can be challenging. Plan Management is a service that is included in your plan whereby a Plan Manager can manage your NDIS funding.

Having a Your Life Disability Support Plan Manager means, we can help you keep track of your support services and ensure that your budget goes to where it’s needed the most. Our services include; Administration tasks, making payments to providers, expense claims processing, tracking of budgets, providing monthly statements for participants and claiming for payment from NDIA.

If you are unable to use public transport including bus, train, tram, ferry or any other transport vehicles due to your disability, Your Life Disability Support are equipped to organize alternative transport methods. This may include taxi subsidy scheme, car rental, rideshare etc. As part of NDIS’s funding scheme, we are able to organise these travel expenses and suitable transport methods for your needs. Although this scheme does not provide funds to include your family members, you can enjoy the convenience of private transport. NDIS provides funds for travel arrangements to people with disabilities in order to achieve individualised goals, objectives and aspirations.

Do you have difficulty performing your daily household tasks? At Your Life Disability Support, we provide assistance to carry out your everyday household tasks. We also assist you to build proper skills in order to perform such tasks. These household tasks include – meal preparation and cooking, food delivery, shopping and banking, basic home and yard maintenance, help to use any accessible equipment for daily tasks. We want our participants to perform their daily household work as autonomously as possible, which is why we also provide, hands on training with the aim of developing skills to perform daily household works etc. Please contact us for more details.

If you are from a culturally and Linguistically Diverse background, in which English is not your mother language, we are equipped to provide an interpreting service to communicate and implement NDIS support plans. As part of the services we provide, Your Life Disability Support will coordinate you with a National Translation and Interpreter Service (TIS) where it is considered necessary. TIS support can also be included in your NDIS support plan. This support can be provided during face to face or over the phone mediums of communication with yourself or family members.
Does your disability affect your access to your vehicle? Do you know that NDIS has special funds for modifying your vehicle so that it can be more accessible? This includes installation of accessible equipment or making certain modifications so that you, as a participant can gain access to it. It also allows you to make modification to it that will eventually allow you to drive your vehicle if you have a valid driver’s license. Vehicle modification enables you to get in and out of the vehicle with or without a wheelchair. It can be easier to carry your wheelchair in your vehicle devoid of heavy lifting. Special equipment can be attached to make transport safer while sitting in your wheelchair in the vehicle. Specialised controls or other devices can also be installed do drive the vehicle. At Your Life Disability Support, we can organise such modifications with proper assessment under NDIS legislation.
At Your Life Disability Support, we understand how difficult it would be to perform your daily living activities if you suffer from a disability. Our goal is to provide support in your daily tasks. We have a range of personal development programs to help you overcome your physical challenges of carrying out day-to-day life activities. If might be challenging for you to dress-up, cook or consume meals, study, transportations and other daily life tasks. We want to increase your ability to live as autonomously as possible. We can provide daily living and life skill development training for some of these tasks such as – food and nutrition, overall cleanliness, communicating with others, accessing transport services, social skills, general hygiene maintenance etc. We also assist family members or carers to explore these issues and help manage them by outlining the proper strategies.
Do you live in a house with other people? Does your disability affect your shared accommodation environment? At Your Life Disability Support, we can help you with everyday tasks, provide you with proper guidance to develop the skills you need to live as independently as possible. We can also provide respite care at any time of day or night. It allows your carer to take a break, study, work, or go to appointments. We provide assistance with and supervising daily tasks in a shared living environment on temporary or ongoing basis. We focus to develop such skills so you can learn to live in a shared environment as autonomously as possible.

Your Life Disability Support will endeavor to assist people with disability to participate in Group-based community, social and recreational activities. We also provide support with engaging in community, social and recreational activities in a one on one or group-based setting. Such social and recreational activities can also be carried out through a Centre-based environment. We specialise in combining tailored individual supports with interactive group activities within our premises.

Our Allied Professionals work with young children, teenagers, adults and their families to provide highly individualised  supports. Our team will help you engage in supports including Occupational Therapy, Speech Therapy, Physiotherapy and Early Childhood Intervention. Our therapists work around the clock and after hours to ensure your needs are being met. Supports can be provided in-home or at our clinic.

Your Support Coordinator will be the link between you and the services in your plan that will help you achieve your goals and enhance your life skills.

They will organise and implement all of the supports in your plan including informal, mainstream, community and funded supports.

Your Support Coordinator will help you develop the skills you need to be able to understand and manage your NDIS plan on your own.

Your Support Coordinator can:

  • Help develop skills and resilience
  • Negotiate services with providers Liaise with providers on your behalf
  • Arrange any required assessments
  • Provide value for money from funds
  • Help you prepare for your plan review
  • Teach you how to use the NDIS portal
  • Connect you with supports in your plan
  • Enhance your ability to manage your plan
  • Address issues that may arise on your behalf
  • Work with you to create budgets for each support
  • Help you understand your plan and service agreements

Physical activity is important for everyone, no matter what their limitations. For many people with a disability, exercise can seem overwhelming and difficult. A National Disability Insurance Scheme (NDIS) registered Personal Trainer can be the perfect solution. At Your Life Disability Support our qualified personal trainers at Unleashed Fitness PT works with the NDIA across Australia to implement NDIS funded Personal Training Sessions to participants who qualify.
